Insurance
Please consult with the human resources department of your insurance provider to determine whether your employee extended benefit plan includes coverage for Clinical Counselling services provided by registered therapeutic counsellors (RTC). My account number is 2641.
Payment
Payment will be made at the time of service by either cash, credit card or email- transfer. Receipts are provided and may be eligible for submission on your income tax as well as your Benefits may reimburse you.
Cancellations
If you need to cancel your appointment, I kindly ask that you reach out at least 48 hours in advance. This way, I can accommodate others who may be waiting for a time slot. If I don’t receive notice, there may be a full charge for the session. Also, please note that if there are three missed appointments without advance notice, we might need to discuss your ongoing counseling. Thank you for your understanding!

To ensure a smooth start to our sessions, please complete the Informed Consent Form and the Confidential Intake Form prior to our first meeting. You can find the forms through the links below. Feel free to either print them out and bring them with you, or scan and email the completed forms to me directly. If you prefer, I’ll also have the forms available for you at our first session.
